WHAT IS ERECTILE DYSFUNCTION?

Erectile Dysfunction (ED), also referred to as impotence, is a prevalent condition that impairs a man's ability to attain or sustain an erection during sexual intercourse. Research on sexual performance and aging men indicates that nearly 50% of men encounter sexual performance difficulties by the age of 50, and this percentage increases by 10% per decade. Therefore, by age 70, nearly 7 out of 10 men experience ED. While ED is not a life-threatening condition, it could be associated with more severe health conditions, such as systemic vascular disease, heart disease, renal failure, prostate cancer, depression, among others.

What causes Erectile Dysfunction?

Erectile dysfunction occurs when a man cannot achieve or maintain an erection during sexual activity due to insufficient pressure in the penile erection chambers. While sexual stimulation usually triggers an erection, men with ED may struggle to maintain an erection until ejaculation. Factors that contribute to ED include age, psychological issues, medical conditions, and certain medications. Younger men with situational ED may experience restricted blood flow due to stress and anxiety, while persistent ED may be a result of underlying issues with nerves, blood vessels, blood pressure, or hormone production. Although Erectile Dysfunction (ED) is often linked with men over 40, it can affect males of any age. There are various causes of erectile dysfunction, which means that there are a wide range of treatment options available. Some of these treatments are simple and involve taking medication or learning stress reduction techniques. In other cases, a more comprehensive approach is required, which involves working closely with a primary care provider to manage one's overall health. Here are some treatment options to consider

Extracorporeal Shock wave Therapy 

ESWT: Low-intensity extracorporeal shock wave therapy (LI-ESWT) is a novel modality that has been developed for treating Erectile Dysfunction (ED). LI-ESWT aims to restore the erectile mechanism in order to enable natural or spontaneous erections. Shock waves (SWs) are acoustic waves that carry energy and when propagating through a medium, can be targeted and focused noninvasively to affect a distant selected anatomical region.

Potential Side Effects: Side effects from ESWT are generally limited to mild bruising, swelling, pain, numbness or tingling in the treated area, and the recovery is minimal compared with that of surgical intervention. Most patients take a day or two off after treatment but don't require a prolonged recovery period.

Oral/Sublingual Medications
Oral/Sublingual Medications: Sildenafil (Viagra), Vardenafil (Levitra), Tadalafil (Cialis), and Avanafil: Oral or sublingual medications work directly on the blood vessels, causing the arteries to the penis to expand. It will cause an erection, only when the man is sexually aroused.

Potential Side Effects: Headaches, facial flushing, backache, congestion, upset stomach, bluish tinge to vision or abnormal vision, hearing loss, and prolonged erections (priapism). These drugs cannot be taken with nitrates due to the possibility of decreased blood pressure. Caution should be used with Alpha blockers for prostate enlargement and/or high blood pressure due to the possibility of decreased blood pressure. May cause an erection.
Vacuum Therapy
Vacuum Therapy: (a cylinder placed over the penis): By withdrawing air, a vacuum is created, mechanically enhancing the flow of blood into the penis. A rubber ring is placed at the base of the erect penis to trap blood and maintain the erection.

Potential Side Effects: Bruising, skin irritation and abrasions, discoloring of the penis, thickening of skin, vascular damage, pain, and diminished ejaculation. May cause an erection.
Injection Therapy
Injection Therapy: Tri-Mix, Caverject, Alprostadiol, Papaverine, Phentolamine: During this painless process, the patient uses a small needle to inject a powerful muscle relaxant into the base of the penis. The relaxation of the muscle tissue allows blood to flow into the erectile tissues in the penis, creating an erection.

Potential Side Effects: Pain, bruising and tenderness, scarring, bleeding, abnormal liver enzymes, and prolonged erection (priapism). May cause an erection.
Penile Implants (outside provider)
Penile Implants: (requires a surgical intervention): Two inflatable balloons are implanted in the penis, as well as a pump into the scrotum and a reservoir near the bladder. When the pump is inflated, fluid from the reservoir flows into the inflatable balloons; creating an erection. (this treatment option available via referral to outside provider)

Potential Side Effects
: Pain, inflammation, malfunction. May cause an erection.
Intraurethral Pellets (outside provider)
Intraurethral Suppositories (MUSE): The patient uses an applicator to insert a small pellet of medication into the opening of the penis. The medication causes blood vessels to relax, so the penis fills with blood and becomes erect. (this treatment option available via referral to outside provider)

Potential Side Effects: Pain, burning sensation, and prolonged erection (priapism). May cause an erection.
Autologous Regenerative Therapies
Intraurethral Suppositories (MUSE): The patient uses an applicator to insert a small pellet of medication into the opening of the penis. The medication causes blood vessels to relax, so the penis fills with blood and becomes erect. (this treatment option available via referral to outside provider)

Potential Side Effects: Pain, burning sensation, and prolonged erection (priapism). May cause an erection.
